Preface HIV Medicine 2006, a medical textbook freely available over the Internet, was downloaded more than 50,000 times in a year. At a cost of 52 € per copy, the added value exceeded 2,500,000 €. The free Internet publication of Tuberculosis 2007 (70 €), which was downloaded more than 80,000 times in 52 weeks, generated more than 5,000,000 Euro in book value in a single year. Hepatology 2009 was downloaded 50,000 times in 2 months and is on track to break all records. These examples demonstrate that the economic implications of open-access publishing are delightfully fascinating. Free medical textbooks contribute to improve medical care. Amedeo Challenge will help bring together institutional sponsors and physicians willing to produce the textbooks.
